Atlas helps eligible Medi-Cal members navigate housing challenges - from finding a place to live, to getting settled and staying securely housed. We help coordinate available housing support and guide you through the process.

What this usually feels like

Formally, this service is Community Supports (CS). You do not need to know the program name to ask for help.

We help you search for housing options, complete applications, gather documents, and navigate the steps toward finding a place to live.
Getting moved in
For eligible members, we help coordinate approved move-in support such as security deposits and other covered housing-related costs.
Home modifications
We help coordinate covered home modifications, such as ramps, grab bars, and other accessibility improvements that can make your home safer and easier to navigate.
Rent assistance
Short-term rental support for members at risk of eviction or moving out of an institutional setting — bridging the gap until stable income returns.
Recuperative & post-hospital care
A safe place to recover after a hospital stay when going home isn't an option — preventing readmissions and giving you time to heal.
Personal care & in-home help
Help with daily tasks, household needs, and caregiver respite so members can stay independent and well in their own homes.

Community Supports are optional services. Each has its own eligibility rules, requires health plan authorization, and depends on local availability and member choice.
