The situation.

Angela, 26, delivered her first baby via unplanned C-section. At the 6-week home visit, she screened at PHQ-9 = 18 with passive suicidal ideation and reported that she 'didn't feel like anyone's mother.'

The nurse who screened me didn't just hand me a phone number. She stayed until I had an appointment on the calendar.

    Week 6 — Screening + safety

    Home-visiting nurse administered PHQ-9; safety plan built with Angela and her partner; therapy scheduled for Week 7.

    Weeks 7–12 — Perinatal therapy

    Weekly perinatal-specialized therapy; medication consult offered; peer group placement.

  3. 03

    Weeks 13–20 — Remission

    PHQ-9 dropped to 3; therapy stepped down to biweekly; relapse-prevention plan documented.

  4. 04

    Weeks 21–52 — 12-month continuity

    Monthly check-ins through the extended postpartum coverage window; well-child visits coordinated.
