Tanisha entered pregnancy with a history of severe preeclampsia in her prior delivery, which had ended in an emergency C-section at 32 weeks and a 3-week NICU stay for her first child.

Trimester 1 — Risk stratification
Comprehensive assessment, low-dose aspirin initiated per ACOG guideline, and MFM co-management arranged.

Trimester 2 — Monitoring cadence
Home BP cuff placed; weekly readings triaged; medically-tailored meal delivery for cardiovascular support.

Trimester 3 — Early intervention
Rising BP trend caught at 36 weeks; planned admission at 37 weeks; vaginal delivery without complications.

Postpartum — 12-month follow
Home visits at weeks 1, 2, 6, and month 3; PPD screening at every visit; lactation and infant care support.
